Client Overview
Visiting Nurse Service of New York (VNSNY ) is one of the
largest home healthcare providers in the United States. The
company provides a wide range of home healthcare services
to more than 112,000 patients throughout New York City,
as well as Nassau and Westchester counties. Lillian Wald,
the founder of public health nursing in the US, established
VNSNY in 1893. The companyâ??s 9,600 care providers make
more than a million professional home visits each year.
Client Objective
Glyphic Technologies, Inc. was asked to re-engineer the
current paper and fax processes surrounding patient files
and to develop a patient centric portal. Our goal was to
provide accelerated integration of disparate applications
by improving availability of integrated clinical information
to physicians. This secure solution enables a staff friendly
interface with role based views of patient data across clinical
and hospital information systems. By developing highly
customized, task oriented tools for physicians through the
patient centric healthcare portal, VNSNY will reduce medical
errors and costs. Furthermore, it allows for increased
use of IT systems, thereby improving employee productivity.
Glyphic Solution
Glyphic worked with VNSNY to develop its Physician Portal
which allows doctors to view and modify patient health
records online pulling data from disparate systems. The
Electronic Health Record (EHR) currently encompasses the
following portlets: Plan of Care (POC), Patient Info/Demographics,
Patient Diagnosis, Patient Medications, Current
Medications + Discontinued Medications, Patient Insurance,
Patient List, Clinician Info, and Action Items.
Search Functionality
Glyphic Technologies, Inc. also designed a search tool that
indexed all content and documents at the firm, making all
Microsoft Word and Adobe PDF documents searchable on
the SharePoint Portal. The search tool is able to generate
context based reports, similar to Westlaw and Lexis search
engines, highlighting terms and ranking them by relevance.
The current system was designed to allow physicians to log
into the portal which brings the physician to the patient
list and the list of action items. The physician can either
select a patient or an action item. If an action item is
selected, he/she will be taken to that particular page (i.e.
an action item may be a POC change that needs to be
â??signedâ?? by the physician. Selecting this will take them to
the POC page.) If a patient is selected from the patient
list then the physician will be able to view patient data
within the other portlets. If the physician attempts to
click on one of the other pages without first selecting a
patient from the patient list, they will see a â??Please select
a patientâ?? message displayed with the portlet. All of this is
architected, developed and deployed to be compliant with
HIPPA standards.
